Medical Social Worker


Purpose:  The Medical Social Worker is a -qualified professional person who provides social case work and prepares social histories for patients and family receiving Nursing or other services from the agency and who have problems in social functioning as it relates to their health and well-being.

Scope & Objective:


Graduate with a Master's Degree from an accredited school of social work.

  1. One (1) year of social work experience in a health care setting.
  2. Effective with both verbal and written communication.
  3. Possess adequate and effective interpersonal skills.

Reports to:  The Director of Patient Clinical Services and designated nursing supervisor.

Supervisory Responsibilities:  This position has no supervisory responsibilities.

Physical Demands:

While performing the duties of this job, the employee is regularly required to talk and hear. The position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift or move items weighing 20 pounds.

Travel:  This position requires travel within the front Range of Colorado.


Tasks/Duties & Job Responsibilities: 

  1. Complies with ethical and professional standards of conducts as set forth by the National Association of Social Workers.
  2. Provides rehabilitative and supportive casework with the purpose of restoring patients to their optimum level of social and health adjustment. This includes assisting patients and their families to understand, accept and adhere to medical regimens.
  3. Assist patients and their families with personal and environmental difficulties which predispose them to illness and interfere with their obtaining maximum benefits from medical care.
  4. Prepares a social history and Plan of Care with the purpose of improving the scope and quality of existing services or extending services. When indicated, assists in the development of the Physician's Plan of Care.
  5. Assist patient and family to utilize existing resources in the home environment or refer patient and family to community agencies and act as an intermediary for the patient as necessary.
  6. Assess, when appropriate, a family's financial situation, taking into consideration patient's prognosis and medical needs and facilitating the referral process to any agency for financial assistance.
  7. Consults with the Physician and other members of the health team for the purpose of assisting them to understand significant social, emotional and environmental factors related to the patient's health problems.
  8. Records clinical/progress notes in the patient's clinical record weekly.
  9. Participates in staff development activities.
  10. Plans for patient's discharge, prepares discharge summaries and instructions.
  11. Attends Paramedical Service meeting and combined Nursing and Paramedic In-Service meetings in the agency and with other agencies.
  12. Performs related and other duties as required.
  13. Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.
  14. Other duties / responsibilities that support the agency's core values of Commitment, Advocacy, Respect and Excellence.
  15. Completes emergency preparedness training as identified as appropriate for the employee level.
  16. Participates in exercises and drills for emergency preparedness as required.
  17. Other duties and tasks as assigned by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.
