Licensed Practical Nurse

This is a Full-time, Benefits Eligible position.  Schedule will be Tuesdays through Saturdays.

Coverage area for this position is Northwest Metro and Greater Boulder.

Purpose:  The Licensed Practical Nurse provides direct­ nursing care to clients as prescribed by the Physician, and under the direction and supervision of the Registered Nurse. Nursing activities are limited to those in compliance with the State LPN/LVN Practice Act, any applicable licensure/certification requirement(s) and agency policies and procedures.

Scope & Objective:

Qualifications:

  1. Graduate of an approved School of Practical/Vocational Nursing.
  2. Must be currently licensed (in good standing) in State(s) of nursing practice.
  3. Preferably one (1) year of experience as a professional nurse.
  4. Documentation of good physical condition to allow standing, bending, stretching and walking.
  5. Good emotional health.
  6. Must meet educational/experience requirements outlined in the laws of licensure states.
  7. Must be able to verbally communicate in the language of the client and document all client related activity. Practice and possess adequate written communication skills as well.
  8. Possess adequate and effective interpersonal skills.

Reports to:     Director of Nursing and/or Designated Nursing Supervisor

Supervisory Responsibilities:  This position has no supervisory responsibilities.

Physical Demands:

While performing the duties of this job, the employee is regularly required to talk and hear. The position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift or move items weighing 50 pounds.

Travel:  This position requires daily travel within the front Range of Colorado.

Tasks/Duties & Responsibilities:

  1. Provides direct skilled nursing care.
  2. Prepares equipment and materials for treatments, practicing aseptic/sterile techniques as required, and assists the client in learning appropriate self-care techniques.
  3. Administers medication as prescribed by the Physician and as permitted by Law.
  4. Assists in the evaluation of effectiveness of treatments and monitors client/patient's reactions.
  5. Contributes to the coordination of services in conjunction with other members of the health care team.
  6. Maintains accurate records.
  7. Reports significant findings or changes in the client/patient's condition to the Registered Nurse and client/patient's Physician.
  8. Records clinical/progress notes in client/patient's clinical record no less often than weekly
  9. Performs related and other duties as required.
  10. Complies with agency Policies and Procedures as well as Employee Handbook detailing Human Resources policies.
  11. Other duties / responsibilities that support the agency's core values of Commitment, Advocacy, Respect, and Excellence.
  12. Completes emergency preparedness training as identified as appropriate for this employee level.
  13. Participates in exercises and drills for emergency preparedness, as required.
  14. Other duties and tasks as assigned by the Optimal Home Care Emergency Command Center communication tree in the event of a local, regional or national emergency and/or disaster.
